Veterans: Update on M21-1 Changes

There’s been a lot of confusion recently about updates to the M21-1 Adjudication Procedures Manual and how they affect secondary claims.

Let’s clear this up:

Secondary claims have NOT been eliminated.
They did NOT change the legal standard for service connection. It is still “at least as likely as not”
They are still granted when a condition is caused or aggravated by a service-connected disability.
What has changed is how the VA is looking at the connection.

The VA is now focusing more on what’s called “but-for” causation, meaning:
Would this condition be less severe but for the service-connected condition?
This actually expands how secondary claims can be argued.

Examples now recognized include:

• When a service-connected condition delays or prevents treatment
• When it interferes with recovery
• When it makes another condition worse, even if not permanently

Another important point:
Permanent worsening is NOT required for aggravation claims.

What has NOT changed:

• You still need a current diagnosis
• You still need a service-connected primary condition
• You still need a strong medical nexus

Bottom line:

This is not a negative change! It just means claims need to be clearly explained and medically supported.
At Schat Healthcare Services, this is already how we approach every DBQ and Nexus Letter:

✔️ Clear medical reasoning
✔️ Causation and aggravation addressed
✔️ Veteran-specific evidence

If anything, this update reinforces the importance of doing these correctly.
