Due to unforeseen circumstances, we regret to announce that we will be closing Beauty Eternal, Eternal Wellness Spa and Emily Hartmann, MD Surgical Practices, effective 11/06/23. Please know that this decision was not made lightly, and it comes with deep sadness as we reflect on the wonderful community and relationships we have built over the years. We have faced many challenges together, but the circumstances we face now have regrettably made it impossible for us to continue our operations.We want to express our profound gratitude to all of you who have been a part of this chapter. Your trust, support, and loyalty have meant the world to us, and we have cherished every moment of serving you. Here are some important details:
Appointment Cancellations: If you have any pending appointments, they will be automatically canceled. If you have pending surgeries we are working hard at contacting you individually. Please do not hesitate to reach out to us if you have any questions or need any assistance. Thank you for your understanding at this time.
Contact Information: Our email and phone lines will remain active for a limited time to address any inquiries or concerns. You can reach us at (530) 487-5020. We will be releasing a list of spa provider referrals at a later date. 💛
Medical Record Release Information: Patients needing assistance in locating another physician should contact their health insurer. With written patient authorization, a copy of the medical record will be available at 1930 Notre Dame Blvd or by calling (530) 487-5020 until 11/10/23. Notice of practice closure will be sent out via mail along with a request for the release of medical records. Requests for records after 11/10/23 can be sent to PO Box 1182 Chico, CA 95927.
