After 20+ years of providing physical therapy to Choctaw and its' surrounding areas, Choctaw Physical Therapy will close today at 5pm.

We are honored to have served the area and contributed to the community throughout the years. Also, we are so grateful that we were entrusted for your physical therapy care both from our patients and their providers.

As a small, privately owned business, times sometimes dictates changes and a reorganization of the company is necessary however difficult of a decision it is to make.

Any patients with active treatment plans please contact your provider. If you or your provider needs records or have billing inquiries please contact our Moore location at Office: 405.759.2700 and Fax: 405.759.2722.

We sincerely will miss the town of Choctaw-the home of our original location.
Please consider our other 2 locations Cutting Edge Physical Therapy at Moore and Yukon.
