Did you know that each Ohio county has a County Board of Developmental Disabilities? These boards help individuals with developmental disabilities and their families access supports and services in their local community.

To get started, families can contact their local County Board of DD to request an intake or eligibility assessment. If eligible, the board can help connect individuals to services such as service coordination, supports for daily living, community inclusion, employment supports, and more.

Accessing DD services early can help individuals build skills, increase independence, and receive support tailored to their needs and goals.

If you’re unsure where to start or need help navigating services, Transitions BH works alongside families and community partners to support individuals through these transitions.

Did you know that Transitions BH participates in OhioRISE?
OhioRISE is a Medicaid program that supports young people with significant behavioral health needs, helping them succeed at home, in school, and in their communities.

“RISE” stands for Resilience through Integrated Systems and Excellence, reflecting the program’s focus on comprehensive, individualized care. Transitions BH is proud to partner with OhioRISE and multiple Care Management Entities (CMEs) across Southwest Ohio to help connect eligible youth and families with the services and supports they need.

Transition-aged youth are teens and young adults who are moving from childhood into adulthood, often between the ages of 14 and 24. For youth with developmental disabilities or behavioral health needs, this time can include big changes—like leaving school services, learning adult life skills, and moving from pediatric to adult care.

During this transition, extra support can help young people build confidence, independence, and emotional well-being, while ensuring families don’t have to navigate changes alone. Services during this stage focus on person-centered planning, skill-building, and coordinated care that honors each individual’s goals, voice, and needs.

A Day in the Life: Before & After Collaboration

Before collaboration
A child with both health and behavioral needs.
Multiple appointments. Different plans.
A parent juggling schedules, repeating the same story, and trying to keep everything straight—alone.

During the day
School has one plan. Providers have another.
Important details get lost. Progress feels slow.
The burden of coordination falls on the family.

After collaboration
Care is coordinated. Communication is shared.
Everyone is working toward the same goals—together.

The result
✔️ Fewer gaps in care
✔️ Less stress for families
✔️ Consistent support across settings
✔️ Better outcomes for the child

When systems collaborate, families don’t have to carry it all. 💙
