Such an informative workshop to learn about what steps need to be made for your future.

Who will make decisions about your care if you are not able to speak for yourself in a medical emergency?
Do the people closest to you know what kind of medical care you want – and don’t want?
Need some encouragement and guidance to get your plan done?

Attend the Enloe Health Advance Care Planning workshop to begin the process of advance care planning (ACP).

• Learn the five easy steps of ACP.
• Understand the different ACP documents like an advance directive and POLST (Physician Orders for Life-Sustaining Treatment).
• Get the documents you need.
• Ask questions and receive answers.
• Complete a worksheet of next steps.
• Play the “Go Wish” game!

The next workshop is scheduled for Sept. 4, 10 a.m. to noon. at the Fountain Medical Conference Room located at 251 Cohasset Road, Suite 105, Chico, next to the Enloe Health Regional Cancer Center.

Meet our Care Coordinator and caregiver, Austin Gerbert! He grew up in the Bay Area and moved to Chico for college in 2010. He studied psychology while at CSU Chico. He started working at Happy at Home in 2019. “I love helping make sure we are providing the best care we can for all of our clients.”
