Spread the word, SALTA Direct Primary Care is hiring!

We're growing and looking for individuals who are interested and excited about the future of Advanced Primary Care! Looking for an experienced APP (NP or PA) to join our office in Pontiac at United Wholesale Mortgage.

Advanced Practice Provider (APP) – Physician Assistant or Nurse Practitioner

About SALTA: SALTA Direct Primary Care, a local Michigan primary care provider, was established in 2001 when we recognized the severe
shortcomings of our healthcare system. We encountered a healthcare system that everyone required but was unaffordable for all.
Essentially, we were overpaying for healthcare and receiving inadequate services. This prompted the need to completely rethink
healthcare delivery and reconstruct primary care from scratch.
SALTA Direct stands out because we work for our clients, not the insurance company. Our technology-driven and experienced team of
medical doctors, nurse practitioners and physician assistants are dedicated to fostering strong relationships with our members. The
direct primary care business model delivers an exceptional healthcare experience with extraordinary outcomes, all while reducing overall
care costs by up to 20%. Headquartered in Troy, Michigan, we extend a warm welcome to both individuals and corporate clients. Our
services are accessible across multiple locations, including Auburn Hills, Clarkston, and Shelby Township, and through our partnership
with United Wholesale Mortgage (UWM). It is our pleasure to serve others with humility, respect, and gratitude. Our success lies in nurturing
meaningful relationships and upholding integrity in all aspects of our work. With the courage to innovate, we strive to make healthcare
affordable, accessible, and exceptional for all.
Location: On- site at SALTA in United Wholesale Mortgage
Terms: Full-time salaried position – salary based on size of member panel and merit – 4-day work week in office with 1 day out of office
but accessible to your panel members.
Accountability Chart: Reports to Office Manager
Qualifications: The APP shall be a graduate of a certified program, shall have passed the national certifying exam, and shall have a
current license from the state of Michigan.
JOB FUNCTIONS: Maintaining clinical services standards is one way to ensure our organization is achieving continued service
excellence. The APP will Interface on a day-to day basis, with clinical and administrative staff to treat and evaluate members under the
supervision of a licensed physician.
• Obtain and record history and physical examinations. Histories and physicals must be reviewed and signed by the supervising
physician.
• Treat and evaluate members with and without the physician present including but not limited to acute care, chronic care,
procedures, in-office and virtual consultations.
• Reduced total member panel of 800, with the expectation of seeing 9-14 members a day.
• Timely member communication within 48 hours of receiving any laboratory or imaging results.
• Evaluate and integrate new technology to empower the Advanced Primary Care model.
• Establish the health coach infrastructure to engage in population health management activities inclusive of care
coordination, closing gaps in care, claims analytics, population risk stratification, and managing/analyzing social
determinants of care, integration of behavioral health and weight management strategies.

• Responsible for several activities related to the delivery of Advanced Primary Care and clinical services such as clinical
systems improvement, quality assurance, and medical protocol development.

Direct primary care (DPC) is known to improve the health of employees while giving municipalities a more predictable way to manage healthcare costs.

In 2015, New Jersey's Union County adopted direct primary care for their municipality to see results such as:

· 38% less in medical expenses than CDHP (consumer driven health plan) participants, yielding annual savings of $1,408,089
· DPC participants incur 37% less in prescription expenses compared to CDHP participants, yielding annual savings of $269,680
· DPC participants spend 46% less out of pocket for prescription and medical expenses than CDHP patients, a $333,639 annual savings
· 73 percent of DPC participants report significant improvement in their overall health since electing the DPC option
· DPC enrollees have at least one chronic condition, while 26 percent have at least two

When looking to curb costs and balance budgets, improve the economic capital of your municipality while improving the human capital of your workers with SALTA direct primary care locally in Michigan.

Since 2001, SALTA is one of the longest running, privately-owned direct primary care companies in the nation. We work for you.
