This April, Accomplished Home Care is proud to recognize Occupational Therapy Month — and the vital role OT plays in the home health continuum of care.

For discharge planners, case managers, and physicians: when you refer a patient for home health, occupational therapy is often the discipline that determines whether they stay home safely long-term.

Our skilled OTs and COTAs work with patients on:

✔️ ADL and IADL retraining
✔️ Home safety evaluations and adaptive equipment recommendations
✔️ Cognitive and fine motor rehabilitation
✔️ Fall prevention and environmental modification
✔️ Caregiver training and education

At Accomplished Home Care, our OT team coordinates closely with the full care team to ensure continuity from hospital or SNF to home — and to keep patients from returning.
