--Allow those worries to be decreased with the help from a dedicated nurse working diligently to help those within the community who need it most!
** I AM NOW EXPANDING MY SERVICES TO ALL NURSING CARE NEEDS!!**
My GOAL is and always will be to teach and educate patients the best way to maintain their independence, live a healthy lifestyle and be safe inside their OWN HOME! I have always believed that the key to success is education.
Whether someone is struggling to remember to take their daily medications, having doubts about the care for their oxygen use within their home or just having difficulties performing day to day tasks. I will teach step by step instructions to help others learn how to live safely within the comfort of their own home.
I will still be providing my Ostomy Care Associate services but, other nursing care needs are readily available. All of my services are individualized SPECIFICLY to the patient and the patient’s needs.
My Services would include:
1. An initial full assessment of the patient and home, if needed (ex. worried about household things increasing risk of falls).
2. Any problems or potential problems discovered on assessment will have short-term interventions created and implemented (tasks will be created that the patient and any caregiver/significate other is encouraged to participate in).
3. An evaluation will be completed after the interventions have been implemented to see if the patients’ health and safety needs were met.
**Any concerns that are out of my scope of practice will be advised for a referral**
