The Director of Home Health Services will supervise, coordinate, monitor and evaluate assigned clinical nursing staff, assessments. Responsibilities also include hiring, scheduling, evaluation, disciplinary actions and termination of employees. The nurse must have the ability to function independently and as a member of an interdisciplinary team and have the ability to exercise independent, sound nursing judgment in planning and providing patient care.
ESSENTIAL DUTIES:
Ensure adherence to agency and department standards and practices.
Assist in and/or develops policies and procedures required for department operations.
Conduct consumer interviews and makes site visits as needed.
Conduct monthly home health aide certification classes.
Render care in accordance with physician's orders.
Observe and monitor patient reaction to medications and procedures.
Orient new staff and train them on policies and procedures.
Acts as a member of inter-disciplinary team to develop acute patient care plans, case management plans and utilization review.
Plan and regularly re-evaluate patient care needs.
Participate in Quality Improvement initiatives.
Ensure the development and maintenance of the clinical record by ensuring that clinical staff documents in a timely manner.
OTHER DUTIES:
Attend scheduled staff meetings, supervision, and on-going training.
Other duties as assigned by management.

JOB REQUIREMENTS:
Graduate from an approved school of professional nursing
Current, valid RN license with Ohio Board of Nursing.
Applicant must have at least (2) years of Home Health Care management & Home health administrative experience.
Have a solid knowledge of Medicare/Medicaid Rules and regulations.
AXXESS software knowledge is a plus.
Two years of home health clinical and clinical supervisory skills.
Reliable means of transportation & current driver's license.
Current, unencumbered state license to practice as a Registered Nurse (RN)
