For many patients the road to recovery takes a little longer than a regular hospital can accommodate. They need more time in a health care facility dedicated to nurturing the person, while treating their complex medical conditions. Grace was the first certified Long Term Acute Care (LTAC) Facility in Cuyahoga County. Our patient average length of stay is 28 days, because they require a well coordinated level of care, over a long period of time, their road brings them here. It is our goal to...
* Bridge the distance between a regular hospital and going home or to a less intensive level of care
* Restore patients highest level of independence
* Have the patient and their family actively participate in accomplishing goals and in the recovery process
* As every patient is different, every care plan is different. By involving our patients and their families we insure that every day is a step in the right direction

As a patient of Grace Hospital, or the loved one of a patient, it is our goal to help you understand the continuum of care from the acute care hospital setting to Grace Hospital, a Long Term Acute Care Hospital (LTAC). We want to provide education to you in regards to both the patients and families expectations. As a family member, or loved one, your involvement in the recovery and healing process is very important, as the average recovery time in an LTAC is usually weeks not days. You provide support to the patient, as do the interdisciplinary team of Grace Hospital, and it is important to keep the lines of communication open. Our physicians and staff work with you by helping you understand your loved one’s prognosis and plan of care during their stay with us.

Friday ends the week of feasting with lunch provided by King David, a post acute care campus featuring independent living, assisted living, skilled nursing care and many other services. The staff enjoyed Donato's pizza and salad. Staff also received hospital t-shirts which can be worn on Fridays.

Two vendors/partners provided meals to the staff today. Champs dropped off Panera bagels for breakfast. Lunch consisted of sandwiches, salads and yummy desserts. Lunch was provided by Respiratory Care Partners and The Heights Rehab and Healthcare Center.

Happy Hospital Week 2024!! Each year Grace celebrates all of our staff during Hospital Week instead of during the weeks designated for each discipline. Each staff member received a Grace Hospital T-shirt in their choice of color that they can wear on Fridays. Today, they were treated with lunch by Smith & Nephew. It is with heavy hearts that we share the sad news of the passing of Willard "Bill" Bartel. Bill was a member of the Board of Directors for over 40 years. During that tenure, he was also the Chairman of the Board for the last 20+ years. His leadership and dedication will be greatly missed.

Our thoughts are with his family and many friends.
