Before the Visit – Preparing for the Visit
• Utilizing planned visits to schedule dedicated chronic disease appointments and identify needed labs, screenings, and care gaps ahead of time.
• Leveraging pre-charting by key practice team members (MAs, RNs, care coordinators) to review charts, confirm diagnoses, reconcile medications, and flag gaps before the patient arrives.
