This webinar will walk you through how practices can shift from a reactive, problem-driven approach to chronic disease management to a more proactive, structured approach. We will frame the content around what happens before, during, and after a chronic care visit, showcasing how planned workflows improve outcomes and efficiency.

Topics Covered

Before the Visit – Preparing for the Visit
• Utilizing planned visits to schedule dedicated chronic disease appointments and identify needed labs, screenings, and care gaps ahead of time.
• Leveraging pre-charting by key practice team members (MAs, RNs, care coordinators) to review charts, confirm diagnoses, reconcile medications, and flag gaps before the patient arrives.
• Ensuring EMR templates contain all guideline-based components for each condition to standardize documentation and reduce missed steps.

During the Visit – Efficient, Coordinated Care
