Did you know? When blood pressure is too high, it is a risk factor for heart attacks and strokes, but when too low, it increases the risks of blood clots and fainting. That is not true with VO2 max with respect to longevity, one of the most notable of these exceptions is maximal aerobic capacity – known as VO2 max.

Not only has it been shown that increasing VO2 max is associated with a reliable risk reduction in cardiovascular mortality, but increasing VO2 max is also associated with a risk reduction of all-cause mortality at any age.

Dr. Carroll is an orthopedic surgeon that treats hip, knee and shoulder arthritis at Movement Orthopedics.

Dr. Carroll: "If you've had multiple treatments for knee arthritis that didn't work you may be a candidate for surgery. Here are some of the recommendations from the American Academy of orthopedic surgeons in regards to treating knee arthritis with surgery:

RECOMMENDATION 1
We cannot recommend performing arthroscopy with lavage and/or debridement in patients with a primary diagnosis of symptomatic osteoarthritis of the knee.
Strength of Recommendation: Strong

Dr. Carroll: "We used to think that this washout procedure was beneficial. However, it does not work very well."

RECOMMENDATION 2
We are unable to recommend for or against arthroscopic partial meniscectomy in patients with osteoarthritis of the knee with a torn meniscus.
Strength of Recommendation: Inconclusive

Dr. Carroll: "Although treating the meniscus problem may help, the arthroscopic procedure does not address the arthritic problem. You may end up in the same or worse pain."

RECOMMENDATION 3
The practitioner might perform a valgus producing proximal tibial osteotomy in patients with symptomatic medial compartment osteoarthritis of the knee.
Strength of Recommendation: Limited

Dr. Carroll: "this is a procedure where we realign the knee to unload the bad part of the knee. This works in younger patients with limited arthritis."

RECOMMENDATION 4
In the absence of reliable evidence, it is the opinion of the work group not to use the free-floating (un-fixed) interpositional device in patients with symptomatic medial compartment osteoarthritis of the knee.
Strength of Recommendation: Consensus

Dr. Carroll: "This is an older procedure that is not done much anymore. It has a very high failure rate and usually requires revision surgery."

When is knee replacement recommended?

Dr. Carroll: "This is a decision made between the patient and the orthopedic surgeon. If conservative treatment has failed and knee arthritis pain is preventing you from doing normal activities, you may be a candidate for knee replacement. Partial or full knee replacement is a highly successful procedure. It can be done on an outpatient basis. You can usually return to normal activities in 4 to 6 months after the surgery. Although there is no surgery with 100% success rate, knee replacement is one of the most successful surgeries that I perform."

Dr. Jeffrey Carroll is an orthopedic surgeon specializing in knee, hip and shoulder surgery.

Dr. Carroll: "I get asked a lot about treatment for knee arthritis. Knee replacement is always the last option. Here are the recommendations from the American Academy of orthopedic surgeons. These are the guidelines that I follow."

RECOMMENDATION 1
We recommend that patients with symptomatic osteoarthritis of the knee participate in self- management programs, strengthening, low-impact aerobic exercises, and neuromuscular education; and engage in physical activity consistent with national guidelines.
Strength of Recommendation: Strong

At Movement Orthopedics we can give you some instruction and education on how to get started

RECOMMENDATION 2
We suggest weight loss for patients with symptomatic osteoarthritis of the knee and a BMI ≥ 25.
Strength of Recommendation: Moderate

The safest and most reliable way to lose weight is by fasting and decreasing your carbohydrate intake. You should coordinate your weight loss program with your primary care provider.

RECOMMENDATION 3A
We cannot recommend using acupuncture in patients with symptomatic osteoarthritis of the knee.
Strength of Recommendation: Strong

Although acupuncture has been proven useful in certain conditions, it is not useful for treating knee arthritis.

RECOMMENDATION 3B
We are unable to recommend for or against the use of physical agents (including electrotherapeutic modalities and magnets) in patients with symptomatic osteoarthritis of the knee. Strength of Recommendation: Inconclusive

RECOMMENDATION 3C
We are unable to recommend for or against manual therapy in patients with symptomatic osteoarthritis of the knee.
Strength of Recommendation: Inconclusive

RECOMMENDATION 4
We are unable to recommend for or against the use of a valgus directing force brace (medial compartment unloader) for patients with symptomatic osteoarthritis of the knee.
Strength of Recommendation: Inconclusive

I have utilized this bracing technique with some success. However, it is not 100%

RECOMMENDATION 5
We cannot suggest that lateral wedge insoles be used for patients with symptomatic medial compartment osteoarthritis of the knee.
Strength of Recommendation: Moderate

RECOMMENDATION 6
We cannot recommend using glucosamine and chondroitin for patients with symptomatic osteoarthritis of the knee.
Strength of Recommendation: Strong

RECOMMENDATION 7A
We recommend nonsteroidal anti-inflammatory drugs (NSAIDs; oral or topical) or Tramadol for patients with symptomatic osteoarthritis of the knee.
Strength of Recommendation: Strong

This is typically how your knee arthritis will be treated initially by your primary care doctor.

RECOMMENDATION 7B
We are unable to recommend for or against the use of acetaminophen, opioids, or pain patches for patients with symptomatic osteoarthritis of the knee.
Strength of Recommendation: Inconclusive

Sometimes will try these medication's if nothing else is working.

RECOMMENDATION 8
We are unable to recommend for or against the use of intraarticular (IA) corticosteroids for patients with symptomatic osteoarthritis of the knee.
Strength of Recommendation: Inconclusive

This is another option that we will sometimes offer if nothing else seems to be working.

RECOMMENDATION 9
We cannot recommend using hyaluronic acid for patients with symptomatic osteoarthritis of the knee.
Strength of Recommendation: Strong

These are injections based on chicken cartilage. We've used them in the past but have recently found that they do not work well. However, it is an option if nothing else is working and the patient understands the possibility of the treatment not working well.

RECOMMENDATION 10
We are unable to recommend for or against growth factor injections and/or platelet rich plasma for patients with symptomatic osteoarthritis of the knee.
Strength of Recommendation: Inconclusive

This is a newer treatment option. It is experimental. We offer this at Movement Orthopedics however, it is expensive and not covered by insurance.

RECOMMENDATION 11
We cannot suggest that the practitioner use needle lavage for patients with symptomatic osteoarthritis of the knee.
Strength of Recommendation: Moderate

This is an older technique to treat me arthritis. There does not appear to be value in offering the treatment.
