Community Wellness Partners

Our Vision: To inspire and nurture successful aging Our Mission: Community Wellness Partners is a faith based non-profit organization that empowers individual wellness through the provision of exceptional healthcare, housing and community services.

LutheranCare® began in 1919 as a vision of one local woman who wanted to offer care and support to the elderly in her community. This dream became a reality when the Lutheran Home for the Aged and Infirm was organized. The original Victorian farmhouse consisted of 13 rooms and provided services to 10 individuals. In 1932, a wing was added to include 14 additional bedrooms and an infirmary. Today, LutheranCare® is a multi-faceted organization providing a broad spectrum of quality programs and services to older adults in need of compassionate care. This includes: •Full-featured residence for older adults •Skilled nursing facility for comprehensive medical care •Special, secure care unit for persons living with Alzheimer's and other dementias •Social model adult day services •Full-time chaplain, on-campus chapels and an abiding respect for all faiths •Short-term and Outpatient Rehabilitation staffed by skilled professionals providing quality rehabilitation services •Helping Hands Home Companions

Presbyterian Homes & Services is a member of Community Wellness Partners – a faith-based, 501(c)(3) nonprofit affiliation of LutheranCare® and Presbyterian Homes & Services – offering the most complete continuity of health and wellness services for older adults in Oneida County. Presbyterian Homes & Services’ 62-acre campus offers a full range of health care services for older adults, including home care and adult day programs, short-term and outpatient rehabilitation, assisted living and skilled nursing. Our secure campus provides the tranquility of nature with its colorful flower gardens and plentiful wildlife, and is close to shopping, restaurants, churches, banks and medical offices. In the beginning, the Utica Presbytery wanted to build a residence for senior Presbyterians in need of nursing care. In 1967, Presbyterian Home for Central New York welcomed its first resident. By the fall of that same year, it was full. Plans for a special needs residence to serve those with Alzheimer’s and other dementias were realized in 1986. Soon after, an adult day health care program opened in 1988. In 2001, our Parkinson’s Residence opened – one of the first in the nation located in a skilled nursing residence – and serves as a model for other facilities across the nation. In 2006, we welcomed the first resident to the telemedicine program. This program is offered for people with Parkinson’s disease and other movement disorders who could benefit from consulting with a specialist via a telecommunications system within the facility. This program serves individuals in six surrounding counties. In 2010, plans were approved to convert 18 Adult Home accommodations to 18 Assisted Living accommodations to provide services to residents that are medically eligible for placement in a nursing home, but due to a stable medical condition, can be cared for in an environment that fosters greater independence. After being awarded a grant in 2011, the Parkinson’s Residence was renovated and expanded to accommodate the needs of additional Parkinson’s residents. Preswick Glen celebrated its 5th Anniversary in 2012 and underwent many upgrades over the next 5 years, including: • Addition of the formal dining room •Campus walkway •2 apartment models and one cottage model •Cosmetic enhancements throughout the community In 2016, Presbyterian Homes & Services and LutheranCare® formed an affiliation, Community Wellness Partners, offering the most complete continuity of health and wellness services for older adults in Oneida County.
