Our goal for Short term Rehabilitation is to return clients to their highest level of functioning, through a highly personalized rehab program that focuses on goal-setting, skills acquisition , and adaptive lifestyle changes. Working together with clients and their families, physicians and caregivers, Green Mountain consistently creates successful outcomes that have earned us an excellent reputation in and around Chittenden County. For our Long term care and End of Life clients are to provided a safe functioning environment to enhance their quality of life through the aging process.
