Quail Ridge Rehab and Living Center is a 118 bed Skilled Nursing Facility located on Stateline RD Colcord, OK directly beside Siloam Springs, AR. We offer rehabilitation services, providing Physical, Occupational, and Speech therapy and we provide long term care. Quail Ridge has a secured memory care unit. We have the ability to provide respite care and hospice care within our facility. Additionally, our facility provides transportation for our residents to help with transfer to appointments and activities.

Today, we pause to honor and remember the thousands of lives lost in the tragic attacks of 9/11. We hold close the memory of the innocent men, women, and children who never returned home, and we pay tribute to the first responders, service members, and ordinary citizens whose courage and sacrifice shined in the darkest of days.

Though many years have passed, the impact remains. The strength, unity, and resilience shown by our nation continues to inspire us. May we never forget the cost of freedom and the importance of standing together in love, compassion, and hope.

We remember. We honor. We will never forget.

You don’t have to be in nursing to make a difference in a nursing home. Every role matters — and every position offers the opportunity to touch lives.

For over 25 years, Tamera has been a shining example of that truth at Quail Ridge. As our Housekeeping & Laundry Supervisor, she goes above and beyond for anyone who needs a hand.

Through life-changing events, Tamera has shown perseverance, dedication, and an unwavering commitment to our residents. She hardly ever misses a day — and honestly, we cheer for her when she does decide to take a well-deserved day off!

Tamera, you are an amazing human being. Thank you for your hard work, compassion, and 25+ years of dedication to Quail Ridge and the residents who call it home.

Like almost everyone else in NE OK & NW AR.. we had a ton of hail damage from the terrible hail storms this year. We entrusted Anytime Roofing to take care of our building… they made so many repairs & improvements. Our new roof being the largest of the projects. They did an OUTSTANDING job. A huge thank you to Anytime Roofing! 🏠

Mark Grayson kept in contact the entire time & really advocated for our home with the insurance.

🎗 The average age of diagnosis is just 6 years old.
🎗 2 out of 3 survivors live with long-term health problems from treatment.
🎗 Only 4% of federal cancer research funding goes to childhood cancers.
🎗 Most drugs used were designed for adults, not kids.
🎗 Families often face treatment costs of hundreds of thousands of dollars.

💛 Our children deserve more than 4%. More research. More funding. More hope.

Quail Ridge Living Center is a 118-bed nursing home located on Stateline Road Colcord, OK behind the casino. We offer skilled and long-term care services. Our skilled therapy team has the knowledge to progress residents to their highest level of function. Each resident receives an individualized plan of care to help them achieve goals for discharge or even long-term goals if staying in our facility.
