Coldspring Transitional Care

Care as it was meant to be! Coldspring is a community where residents receive the care they need to be social, active, and involved.

Built in the Fall of 2014, Coldspring Transitional Care Center is a destination for those needing rehabilitative or nursing care services on a short term or continuing basis. You’ll find the facility is a tranquil oasis surrounded by professionally-manicured grounds. Coldspring’s amenities are matched by the high level of care and services provided by our in house team of licensed therapists, nurses and caregivers. The facility encompasses both a residential area for patients with longer term needs, and a separate rehab center for individuals requiring short term services. Residents can enjoy many amenities including a chapel, beauty shop, and family guest room. To arrange a personal tour or for other questions, call 859-441-4600.

We had a blast connecting with our community neighbors at the Miamisburg Community Center Health Fair and the PAM Rehabilitation health fair. We were also happy to sponsor Double Dippin Ice Cream for the PAM team to celebrate Rehab Week! We're so proud of their dedication to supporting our local communities.

Strike a pose! Our residents at Dayspring of Miami Valley recently got to experience their own special glamour photo shoot, and as you can see, they rocked it! The laughter and smiles were contagious. We're so grateful to our fantastic activities team for organizing such a special event and for taking these incredible pictures. They truly brightened everyone's day!

Ensuring our community has top-notch emergency care is paramount. Our amazing team at Highlandspring are true heroes! In March, one of our beautiful nurses experienced a sudden cardiac event while at work! 5 amazing nurses sprung into action performing life saving CPR and securing emergency care! Sarah is here because of their heroics and is giving back by advancing her nursing degree! So inspirational! Thanks Heart Mini and American Heart Association for helping us honor these amazing clinicians!!!
