Dear Friends,

We recently launched a survey campaign to gather information about people's health awareness. We appreciate your time and willingness to share your thoughts, which has helped us better understand your point of view.

We promise to send the gift cards out to the users within 3-5 business days of survey completion. However, survey abuse and regulatory issues have slowed gift card distribution. Fortunately, we have discovered a solution to these issues.

-- If you completed the survey but did not receive the gift cards, we appreciate your patience and ask that you please give us some more time to evaluate your submission.

-- If you took the survey and received it. we appreciate your feedback and hope you enjoy the gift card. We'd also appreciate it if you could leave a message to confirm receipt of the gift card in the comments section below.

Best,
