Some surgeons just shouldn't do lower extremity trauma. A few 'air ball' screws and some poor reduction left us wondering what they were trying to accomplish. This poor patient was left with a life altering, crippling ankle after a bilateral lower extremity trauma related to a MVA. The right ankle obviously wasn't reduced well and succumbed to severe, post traumatic arthrosis with AVN. The patient was nearly incapacitated with pain and had been offered a brace or amputation. With restor3D technology, we were able to 3D print a titanium talar cage construct and post talectomy, we retrograde inserted an Orthofix 200mm, 12mm intramedullary rod, Trinity Elite, and BMAC. Mission accomplished in 90mins skin to skin.

When replacements go bad. Old MVA, multiple surgeries following ORIF. Implants subsided. Revised to fusion with a fresh distal tibial graft, opposite tibial BMAC harvest, Trinity Elite, and Paragon 28. Fresh Talus was NOT an available option as the tissue banks had no matches.

Both the medial double arthrodesis and MCDO play a significant role in AAFD correction surgery. We feel that AAFD correction has very predictable and rewarding results when performed correctly and with the right combination of procedures.
