Here’s exactly how to get signed up and what to expect:

👉 Before April 15th:
You can fill out the paperwork in two steps online:

👉 On April 15th:
• As you arrive, you will be given a tag with a number while you are in line
• Doors will open at 10:00 a.m.
• If you haven’t completed paperwork yet, you can do that when you come inside (the online option will also stay open)

Our Sweetwater Clinic lines are still down. We are working to restore them, in the meantime all patients can contact the Rural Health Clinic in Colorado City to reach Dr. Moses and Jade Crawford, FNP-C. Normal office hours are still scheduled for the Sweetwater Clinic.
