Notice of Practice Closing
Dear Patients of Springs Behavioral Health,
We are writing to inform you that our practice will be closing as of August 31, 2025 (“Effective Date”).
This decision was not made lightly. It has been a true privilege to work with so many wonderful patients, and stepping away from these trusted relationships is, without question, the most difficult part. We are deeply grateful for the opportunity to support your mental health journey and thank you for allowing us to be part of your care.
After closing, your health records from our practice will be transferred to Morgan Records Management, LLC (the “Records Custodian”). The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. Your records will be destroyed no less than 7 years after the last date of services you received from us.
If you wish to have a copy of your records sent to you or to another provider or facility, you may submit a written request to our office before the Effective Date. After the Effective Date you will need to submit a request form with the Records Custodian. The release form can be found on their website:

Please Note:
- In accordance with applicable state law, you may be charged for the copying and transmittal of the records.
- There may be a transition period from the time I close to the time Morgan Records Management has access to my charts.
If you have been seen as a patient within the past 6 months, we will provide you with a 90-day supply of non-controlled medications. Patients with controlled medications will be provided with a 30-day supply.
