Rocky Mountain Health Care Services grows with our community’s unique and ever-changing needs. Currently we have one outstanding programs: Rocky Mountain PACE. Rocky Mountain PACE (Program of All-inclusive Care for the Elderly)

The PACE model of care—Program of All-inclusive Care for the Elderly—is considered the gold standard of care for seniors nationwide. PACE is enriching the lives of senior through coordinated care. Seniors who work with our team receive medical and support services to empower them to live as independently as possible in their own home or living community. Our goal is to help seniors maintain the highest level of independence as possible. Rocky Mountain PACE is the only national PACE program in the region, offering a holistic suite of services to meet the health care needs of seniors in our community. Seniors and their families do well if they have a role in their own health and we see ourselves as partners in their wellbeing. From their physical, social and emotional health, to the simple things like transportation and supportive services that help them access their care, we partner with seniors in keeping them as independent as possible. Our seniors experience their health care delivery with a coordinated, holistic and integrated approach. Our team discusses health care treatment options from many vantage points which gives our participants a strong likelihood for healthy outcomes. Rocky Mountain PACE is gaining national attention for our exceptional behavioral health integration, which is not a required discipline to operate a PACE program. We also are actively sought after to provide consulting services to other PACE and health care programs throughout the nation. Our notoriety stems from our exceptional financial stewardship, ability to cut administrative costs and waste utilizing Lean Six Sigma methodology, and our tremendous success in executing a rapidly growing PACE program.

PACE reaches a major milestone: 200 programs nationwide.

With the opening of PACE Northeast Michigan, there are now 200 PACE programs operating across 33 states and Washington, DC—supporting more than 100,000 older adults with comprehensive, person-centered care.

For 55 years, PACE has helped seniors live safely in their homes and communities while preserving independence, dignity, and quality of life. As our nation’s population ages, PACE remains a proven, outcomes-driven model that meets the complex needs of older adults and strengthens families and communities.

Pets can help reduce stress, lower blood pressure, ease loneliness, and even spark cherished memories for older adults.
