Healthy Palmetto recently unveiled its 2025–2029 statewide action plan, targeting six strategic priorities such as nutrition supports, food-is-medicine interventions, community mobility, school and outdoor physical activity opportunities

We are proud to welcome Dr. Silvio Inzucchi as a guest speaker at the 2025 Diabetes Free SC Annual Meeting.

Dr. Inzucchi is a world-renowned endocrinologist and clinical expert in diabetes care. He currently serves as clinical chief of endocrinology and medical director of the Yale Medicine Diabetes Center, with more than 500 publications and extensive experience in leading clinical trials on diabetes treatment and cardiovascular health.

As we mark , we are honored to have Dr. Inzucchi join our partners to share his perspective on the future of diabetes management.

In 2025, Diabetes Free SC will support programs that encourage prevention, care and community-based outreach throughout South Carolina.

The SC MOMs program will grow to serve more communities, offering complete clinical care to help women manage diabetes during pregnancy.

The Women’s Health Initiative will continue to empower women of all ages with the tools and support needed to manage diabetes and bridge those in the SC MOMs program to continue to manage diabetes after pregnancy.

A new program, SHIELD, will launch to bring diabetes education and prevention to adults through trusted, faith-based outreach in West Columbia and Anderson County.

Partners include Prisma Health, MUSC Health, Rural Health Services, Inc., Clemson Rural Health, Generation4 and Brookland-Lakeview Empowerment Center - BLEC.

Preventing diabetes starts well before diagnosis. For many children, that prevention begins at school.

Why focus on schools? Because most children spend the majority of their time in classrooms. That makes schools a powerful place for prevention.

Through support from Diabetes Free SC, Alliance for a Healthier Generation and the MUSC Boeing Center for Children's Wellness are helping districts across South Carolina implement policy, systems and environmental changes.

The SC MOMs program is a powerful initiative helping pregnant women with diabetes get the care they need—when they need it most. Since 2020, it’s helped reduce preterm births by 20% and improved outcomes for both moms and babies across South Carolina.

What SC MOMs delivers:

Without access to school meals, families often struggle to keep nutritious food on the table. That’s where our partners step in.

Diabetes Free SC supports the Alliance for a Healthier Generation, which works with school districts and the South Carolina Department of Education to promote summer meal programs like Summer Break Café — providing free, healthy meals in libraries, parks, churches and more.

Healthy habits don’t stop at the meal site. Healthier Generation also offers family-friendly tools like Snack Switch Up and Flavor Remix to make nutritious eating fun and doable at home.

Last week, Diabetes Free SC presented the success of VeggieRx, a powerful collaboration with FoodShare South Carolina, at the American Society for Nutrition annual conference in Orlando.

Doctors in South Carolina prescribed fresh produce to patients with or at risk of diabetes. With support from FoodShare South Carolina, patients received weekly fresh food boxes. The result?
✅ Weight loss
✅ Lower BMI
✅ Reduced A1C

In South Carolina, 1 in 10 pregnant women have diabetes, which can lead to serious health issues. Our SC MOMs program helps these women manage their diabetes for healthier pregnancies and better outcomes for moms and babies.

Since 2020, our partners have helped over 3,000 women, reducing risks like birth defects and preterm birth.

Adults with diabetes face higher risks during surgery. Diabetes Free SC supports the South Carolina Surgical Quality Collaborative Diabetes Initiative (SQCDI), which helps improve health before surgery to reduce these risks.

Since 2022, 110 patients have successfully completed surgery with the program, and over 300 are currently enrolled.
