Here are just some of the advantages of having us present when your loved one is discharged from the hospital:

➡️ The providers are sharing a lot of information with your loved one very quickly and in a stressful situation. It's likely they will only recall 30% of what was said to them. When we're present, we can find clarity through the medical jargon and take notes so your loved one gets the care they need.

➡️ Once your loved one is home and settled, we will conduct a house call. This allows us to make sure your loved one is taking any necessary medications and functioning properly.

When you next visit your aging loved one, make sure their basic needs, such as eating, bathing, and taking their medications, are tasks they can and are accomplishing.

Other things to look out for in your loved one:
