Heart disease is the leading cause of death for older adults. But here's what the healthcare system often misses: heart health isn't just about your cardiology appointments or your medication list.

Cassie has worked in healthcare nearly her entire life. She has experience as a Pharmacy Technician, healthcare scheduler, home health aide, and residential support assistant. But her passion for caregiving started long before her professional career.

"My grandmother always called me 'Nurse Cassie' as a child. I became her caregiver during her declining health, and now I care for my grandfather. I'm also a mother to two children, one of whom has special needs. Helping people has always been part of who I am."

One of our care navigators works with a client whose major interactions revolve around supporting her mental health.

Here's what that looks like in practice:

▪️When the client goes into the community, the care navigator makes sure it's an environment that will make her feel supported—not overstimulated or anxious.
▪️When she visits practitioners or other professionals, the care navigator goes with her. Not to take over, but to be her constant. Often, the care navigator doesn't even have to say anything; the client just feels better because she is there.
▪️If the client has trouble remembering details during appointments, the care navigator provides historical context. If she needs help navigating the mental health system, understanding the difference between a psychiatrist, psychologist, counselor, LPC, or MSW, the care navigator explains it all.

Because with mental health, first impressions matter. If someone comes in unprepared and the interaction goes badly? That client might shut down entirely.

Mental health is its own specialty, and it's hard to navigate alone. But with the right support, people don't have to.

During a recent roadmap assessment, a care navigator asked a wife about her mood and whether she feels sad most of the time.

Her husband? He was squirming in his seat like she'd just asked something wildly inappropriate. He couldn't sit still. He was visibly uncomfortable. At one point, he asked, "Am I going to be expected to answer these questions too?"

This is the generational divide around mental health.

That generation didn't talk about depression, anxiety, or mood disorders. They pushed through. They kept it private
So when we ask about mental health during our roadmap, discomfort is normal. But we push through it anyway, gently, respectfully, and thoughtfully.

Because here's what's dangerous: assuming that confusion and depression are just "normal" parts of aging.

Here's what typically happens in the traditional healthcare system: Someone's doing okay at home. Then suddenly they have a fall, a hospitalization, or a crisis, and all of a sudden they're being told they need to move to assisted living or a nursing home immediately.

But here's what we know: people don't decline overnight. The signs are there. The small changes happen gradually. But because most healthcare services only check in occasionally. Or only show up when there's already a problem. They miss all the warning signs leading up to that "big jump."

At Navigate Wellness, we don't operate that way.

Some of our clients we see once a month. Others we're with 24/7. But regardless of how often we're there, we're watching. We're adapting. We're catching the small changes before they become crises.

We see when someone starts struggling to get out of their chair. So we add mobility support before they fall.
We notice when medication management is getting confusing. So we step in before they end up in the ER from a missed dose.

We recognize when isolation is creeping in. So we bring in companion care before depression sets in.
Most people want to stay where they are. They hold on a little longer than they should, which leaves them more vulnerable to risks and injury. Then the system reacts with panic, saying, "You can't stay home anymore, it's not safe!" and suddenly they're being moved out of the place they love.

By gradually adapting our care plan as our clients' needs change, we're able to add support a little bit at a time. This allows us to achieve the same level of care that would typically require a facility. Except we're doing it at home, where they want to be.

We're not waiting for the fall to add safety measures. We're not waiting for the crisis to increase support. We're not waiting for things to get so bad that a nursing home feels like the only option.

We're preventing the big jumps altogether.

That's the difference between reactive care and proactive care. That's the difference between being tossed around by the system and staying in control of your life.
