Partial knee replacement ….IS NOT A TEMPORARY PROCEDURE!

For patients with knee arthritis limited to one compartment… partial knee replacement is a long term solution! Just like total knee replacement.

The thought that partial knee replacement is “a bridge” procedure until you need a total knee replacement is outdated thinking. Numerous studies have demonstrated the excellent long term results after partial knee replacement.

This pictures demonstrates the difference between a fixed and mobile bearing partial knee replacement.

You can see with the fixed bearing device, the plastic insert is locked in the tibial tray.

With a mobile bearing implant, the plastic bearing moves independently of the tibial tray. This reduces the stress on the plastic insert, which as a result reduces the risk of wear.

The Oxford Partial Knee replacement has a unique design and is unlike any other partial knee devices.
- It has a mobile bearing that was designed to replace the function of the meniscus in the knee.
- Because the meniscal bearing is mobile, there is essentially no wear of the plastic bearing. This eliminates one of the common reasons for failure, which is wear of this bearing.

You can see in this patient after total knee replacement, both compartments of the knee are replaced.

One of advantage of total knee replacement in this setting is that we can correct the alignment of the knee.

Knees that are ACL deficient, like the one seen here, tend to have more fixed deformities that are not correctable. So it is important to restore the knee alignment to a more neutral position for optimal outcome.

This is not necessary in partial knee replacement, because the ligaments are normal and the knee deformity corrects when you replace the affected compartment.

Not all likes are candidates for partial knee replacement.

With this knee, there is bone on bone medial arthritis. You can also see that there is a significant subluxation of the knee joint. The femur, or top bone, is shifted out of place relative to the tibia, or bottom bone.

This is an indication that ligaments in the are deficient, specifically the ACL.

When this is the case, total knee replacement is the only option. In this setting total knee replacement has excellent outcomes.

Patellofemoral or "knee cap" replacement resurfaces the underside of the kneecap with plastic and the groove that the knee cap tracks on the femur with metal.

Again, this preserves the other normal 2/3's of the knee.

This is the least common pattern of OA, accounting for less than 5% of patients.

In my experience, patients have total knee replacement for this type of OA are typically not satisfied with their knees, because the no longer feel normal.
