Dear Valued Patient,
I want to share some important updates regarding my medical practice. After many wonderful years of caring for this community, I have made the decision to retire. December 5th will be our last day seeing patients.
For patients needing ongoing or new urologic care, I would like to offer a few recommendations of trusted urology providers in the area:

- 1513 Wellness & Weight Loss – (706) 221-4848 This clinic has been fully trained in, and follows, the same hormone replacement treatment protocols for men and women developed by my practice. They will also be offering our patients special pricing.
You may use your patient portal to request medication refills and to access office notes and lab results. Due to the overwhelming volume of medical records requests and our limited remaining staff, we will be adjusting our records fulfillment process as follows:
- At this time, we are able to provide your most recent office notes, laboratory results, and test/diagnostic reports upon request.
- All complete medical charts will be securely transferred to our designated Custodian of Records, Cariend. Cariend.com
- Because of the large number of records involved, Cariend will receive the full charts within 6–8 weeks.
- Once the transfer is complete, you will be able to obtain your full chart from Cariend at no cost.
We encourage you to check our website for updates as we progress through this transition.
Before scheduling with a new urologist, please check with your insurance company to confirm which providers are in your network and covered under your plan. You are welcome to choose any provider you prefer, and we will gladly assist in transferring your medical records to their office when needed.
It has been one of the greatest honors of my life to serve as your physician. The trust you have placed in me, and the relationships we have built over the years, have meant more to me than words can express. I remain deeply grateful for the opportunity to be part of your healthcare journey.
