Our Story
Physicians CareConnection (PCC), formerly Physicians Free Clinic/Voluntary Care Network (PFC/VCN), is the largest coordinator of voluntary health care services in Central Ohio with over 1000 primary/specialty care doctors and 13 hospitals participating. PCC connects low-income, uninsured residents with medical homes and improves access (reduce delays) to specialty care.
The PCC care coordination model address the needs of patients as they navigate the health care system and work to provide them with the resources necessary to obtain appropriate health care services. Some of the programs include:
1. Primary Care: Patients are able to receive primary care through our Monday nights walk-in free clinic, operated at the Columbus Public Health department on; 240 Parsons Avenue, Columbus, OH 43215, beginning at 4:30pm. The clinic serves as a “bridge” or a gateway into the health care system for people who do not have a primary care doctor.
2. Voluntary Care Network: PCC clients are able to access specialty care through a decentralized or “virtual” network of providers.
3. OSU Eye Free Clinic: PCC coordinates the ophthalmology clinic at the OSU Havener Eye Institute for patients with serious eye illness or injury.
4. Dental CareConnection: The dental clinic also operated at the Columbus Public Health department on 240 Parsons Avenue, provides tooth extractions restorative and denture services to those without dental insurance coverage.
5. STEPONE for a Healthy Pregnancy: An Intake and Referral program that connects pregnant women with affordable, timely and convenient prenatal care and resources for a healthy pregnancy.
6. Family Coaching and Support services for pregnant women and people with diabetes
