Our beautiful and highly functional campus is owned and operated by Cardinal Care Management, an Ohio based leader in the rehabilitation and long-term care provider industry. At Creekside Care Center the care for you or your loved one is our only concern. Our campus accommodates 112 rehabilitation and skilled nursing residents. We combine the most current care practices and technologies with our core values of compassion, fairness, integrity and respect for human dignity to offer a positive living environment for those we care for, as well as providing a welcoming work environment and team approach so each employee can be successful.
