Our main goal is to take all necessary actions to ensure the health and safety of our clients and ourselves. With that in mind, Relevate is implementing new practices to comply with COVID 19 Policies from the Ohio State Medical Board, the Ohio Department of Health and OSHA. g you again! Your therapist will be in touch regarding schedules. COVID 19 PREVENTION POLICIES TO ENSURE YOUR SAFETY:
Complying with orders from the Ohio State Medical Board, the Ohio Health Department and OSHA. BEFORE YOUR APPOINTMENT:
• Your body temperature must be taken the day of your appointment. Staff
temperatures will be taken twice per day. Please wash your hands before getting into your car and coming to our office for your appointment. Staff will keep a daily health log.
• If you are feel sick, please do not come in for your scheduled appointment. We will also cancel appointments if we are feeling sick.
• If you have a fever, you must reschedule your appointment. Please wait 10-12 days to reschedule with no fever and COVID 19 symptoms. The following are all possible COVID 19 Symptoms: fever, aches/chills, cough, sore throat, fatigue, headache, shortness of breath, and loss of taste or smell. Your therapist may ask you if you have any of these symptoms before your treatment
• Continue with social distancing policies before and after your treatment, advised by the Ohio Health Department:
*Wear a mask or face covering when you are in enclosed spaces and when you are not within 6 feet of another person.
*Avoid large groups and keep gatherings to under 10 people.
*Maintain good hygiene at all times — wash your hands and sanitize often & avoid touching your eyes, nose and mouth with unwashed hands. SANITIZING/CLEANING:
• As with our normal operations and procedures, clean sheets and blankets will be used for each client, along with a new face mask for each client. Therapists will wash their hands before and after treatments and after touching any used sheets, blankets, and fabric face masks. All laundry is machine washed in hot and soapy water and dried on high heat.
• Additionally, appointments will be scheduled so staff has enough time to san- itize treatment rooms, restrooms and any areas touched by clients and staff between each appointment. We will sanitize with any of the following: Lysol/ Bleach cleaning wipes or spray, or Cavicide wipes or spray, and 70% alcohol wipes on cell phones and tablet devices. We will be closed one day each week to deep clean our office. We will keep a daily and weekly cleaning log. WHEN YOU ARRIVE FOR YOUR APPOINTMENT:
• Wash your hands for 20 seconds or use hand sanitizer upon entering the
building
• All staff will wear a mask during your treatment. We recommend you wear a face covering or face mask, as well. Some exceptions may be made for clients (i.e. claustrophobia, ill fitting mask). Staff will also wear a new disposable medical gown, lab coat or apron for each client session. Or, we will change clothes between each client.
• We will have designated waiting areas of 6 feet apart for you to wait for your appointment. In most cases, we will ask you to wait in your car and text you to come into the building.
• Your therapist will give a brief health and travel questionnaire upon your arrival and have you sign a consent for treatment form.
• In order to allow for better ventilation in our treatment rooms, we will either have the treatment door open with a fan or a HEPA air purifier in the treatment rooms. Ohio Department of Health Covid-19 Tracking:
