Planning for Moving to Adult Health Care

Transitioning to Adult Health Care with Confidence
To help prepare youth and young adults with intellectual and developmental disabilities (ID/DD) for their health care transition, six toolkits were developed under the leadership of Family Voices, in collaboration with the LifeCourse Nexus, SPAN, and Got Transition. This project is supported by the Administration of Community Living, U.S. Department of Health and Human Services. For additional health care transition resources for youth and young adults with ID/DD, their families/caregivers, and clinicians and direct support workers, visit the Center for Transition to Adult Health Care for Youth with Disabilities.

What is a Toolkit?
The six toolkits aim to help you learn and get ready for moving to adult health care and living a healthy adult life. They will give you lots of information and resources to help you during this time of transition. Each toolkit will focus on a different topic about health care transition.

Each toolkit consists of a Side-by-Side Guide, EZ-Reader, tool, and tip sheet.

The Side-by-Side Guide helps you learn, discuss, and take action on health-related topics and includes tips for supporters.
The EZ-Reader provides easy-to-understand information about specific topics in the Side-by-Side Guide using short sentences and pictures.
The tool is an activity to help you prepare for your health care transition.
The tip sheet helps you know how to fill out and use each tool.

Grief Across the Caregiving Trajectory
Grief is naturally a parent’s constant companion, from diagnosis – or lack thereof – onwards, including into bereavement. Psychologist Wendy Lichenthal, PhD is both a grief researcher and a clinician, with a particular expertise in the psychology of cancer for the patient and the family.

During this event, Wendy framed aspects of the grief experience and discussed using meaning and meaning-making to help with coping, regardless of diagnosis.

The Bureau for Family-Centered Services administers a set of services for children, birth to 21 years old, who have or are at risk for a chronic medical condition, disability, or special health care need.

Care Coordination
Provides community-based care coordinators to assist families in accessing needed healthcare and related services for children with chronic illness or disability, including help with finding medical, social, psychological, educational, and financial assistance and resources. Financial assistance is available to eligible families.

Complex Care Clinic*
This is a specialty service for children with chronic and complex health conditions who would benefit from consultative services to address health concerns and educational needs. The one-time assessment can be with an individual provider, a group consultation, or a clinic appointment. The team consists of a developmental pediatrician, physical therapist, educator, and feeding and swallowing specialist, as appropriate. Amoskeag Health conducts virtual or in-person consultations and collaboration with families, medical professionals, schools, and other community agencies caring for children. Nutrition, Feeding and Swallowing Program*
A statewide network of pediatric dieticians and feeding and swallowing specialists providing in-home and community-based consultation. To learn more about this program, call 603-296-9213.

Child Development Clinic*

The NH Food Bank’s mobile food pantry travels to many different areas of need across New Hampshire.
What to know before you go:

Our mobile food pantries are a drive-thru format.
Food distributions take place on a first-come, first-served basis and are while supplies last.
Each household receives an assortment of produce and shelf-stable foods.
We welcome anyone in need of food assistance to join us – no identification or requirements needed.

Our Story

We are a Family To Family Health Information Center assisting all families especially those raising and working with children having special healthcare needs and or disabilities.

NH Family Voices is staffed by parents of children and young adults with special health care needs/disabilities. As parents, we also travel through the maze of services and programs designed to help our kids, so we understand the issues that families face. We provide options and advice, offer a multitude of resources, and tap into a network of other families and professionals to help provide you with support and information.

Our Services include:

♥ Individual assistance by telephone, e-mail and social media (messaging). ♥ Help in understanding how to navigate through systems (health, human services, education, and health care financing). ♥ Assisting families to make better informed decisions about their children’s health care and be better able to participate in systems building activities in their communities and within NH. ♥ Information and resources via mail, e-mail, social media, YouTube, Pinterest and our website. ♥ A private parent to parent Facebook group of over 900 members, many of whom are also maneuvering the systems of care for their children and youth with special health care needs/disabilities. ♥ Trainings for families on topics such as healthcare financing, best practices, family/professional partnerships... and more ♥ A lending library with over 3,000 titles specifically identified for our kids and families. ♥ Parent to Parent support when you just need to talk to someone.
