In-Home Care

In-Home Care In-Home Care is a nonprofit program of Ascentria Care Alliance, providing non-medical services in the home to adults.

In-Home Care provides non-medical services such as transportation, personal care, light housekeeping, medication reminders and meal preparation. We accept private payment, work with Medicaid Choices for Independence, any Long Term Care insurance providers and grants. In-Home Care is a Licensed Home Health Care Agency and a certified Other Qualified Agency (OQA). In-Home Care is a member of Ascentria Care Alliance.

Ascentria’s Young Parent Living Program serves low-income, single, teenage mothers and their small children who might otherwise be homeless, living in unsafe situations, or sub-standard housing. Two years ago my friend Paul Scott and I wrote an op-ed urging our legislators to invest in the Choices for Independence (CFI) waiver. Paul was one of thousands of medically vulnerable people in New Hampshire, ages 18 to over 100, who qualify for nursing homes but choose to remain in their own homes.

Paul suffered a catastrophic injury many years ago and it was through the CFI waiver that he was able to remain at home. Despite our plea, CFI is currently failing people like Paul.

Home and community-based care cost a fraction of what a nursing home does. Unfortunately, these programs have been underfunded for over a decade, so badly that New Hampshire ranks 50th in the amount of Medicaid funding it spends on home and community-based care...

Amy Moore, Director of Ascentria's In-Home Care Program, testified in front of the New Hampshire House on March 17 in support of the Choices for Independence Program.

"Good afternoon, Mr. Chairman and members of the Committee. My name is Amy Moore. I am the Director of Ascentria In-Home Care, which provides personal care services for over 400 people through CFI—the Choices for Independence Program. I also serve on the board of the Home Care, Hospice, and Palliative Care Alliance of New Hampshire.

Thousands of Granite Staters depend on the CFI program. These are low-income, medically vulnerable individuals ages 18 to 100 who qualify for nursing homes, but who choose to remain in their own homes. Home and community-based care cost a fraction of what a nursing home does. This past year brought upon the world a pandemic, shone a light upon a nation still struggling with mistreatment and oppression and brought forth a challenge to our democracy.

Martin Luther King Jr, from a Birmingham jail on April 16, 1963 wrote, “Injustice anywhere is a threat to justice everywhere.” This is where we stand as a team and organization, members of the larger social good community, and members of the human race.

The recent attacks on the U.S Capitol rise in stark contrast to the nonviolent direct action used by Martin Luther King, Jr. to create constructive tension, dialogue, and change.

We believe that as we work under a higher purpose every day for diversity, equity and inclusion that the world can be a better place. We will continue to make ongoing and focused efforts to develop our organization, impact the clients we serve and the communities we work within.

You have the organization’s commitment including Executive and Leadership Teams and Diversity Equity and Inclusion (DEI) Task Force, and focus on making strides by providing an open forum for dialogue, training, and education, enacting company policies that support equality, and bringing that into the work we do as an organization to impact broader systemic problems. We must use the strength of our organization to make change.

In Honor of Martin Luther King Junior, the DEI Task Force wishes to thank Ascentria. Thank you for the openness, support, empowerment, and recognition of the need to hear our employees and envision strategies that improve the experiences of our neighbors and communities.

Caregivers are quiet heroes; they shoulder the daily needs of others with love, resilience, determination, and oftentimes very little public recognition.

At Ascentria In-Home Care we proudly care for our Veterans, their spouses, and surviving spouses every day. Our commitment to their care is just one of the ways we demonstrate appreciation for their service to our country. These heroes – our fathers, mothers, spouses, and friends – may be eligible for a unique benefit through the US Department of Veterans Affairs (VA) called “Pension with Aid and Attendance.”

Millions of eligible Veterans and families go without Pension with Aid and Attendance simply because of not knowing about the benefit and inability to pay for home care. Through our partnership with Veterans Care Coordination, we are able to assist eligible Veterans and their spouses in applying for and obtaining the Pension with Aid and Attendance Benefit through the VA. What makes this partnership unique is the fact that Veterans Care Coordination can offer financial assistance to clients who cannot afford to pay for the up-front, out-of-pocket expense of home care.
