Diablo Valley Primary Care in Concord, CA, is a private practice devoted to delivering high quality and compassionate care to patients in our community.

At Diablo Valley Primary Care, we are committed to the health and well being of our patients. We believe in the promotion of preventive practices, as we recognize that through effective primary prevention and better management of chronic diseases, we can help our patients avert illnesses, and thus, improve their health and their quality of life. Although we are experts at treating diseases, we believe that, as healers, we treat people and not diseases. We utilize shared-decision making, for we are convinced that the best medical decisions are those made between doctor and patient.

Leah is a board certified Family Nurse Practitioner with prior experience as a registered nurse in a variety of settings including acute care and public health. She has worked in San Diego, California; Cleveland, Ohio; and Sydney, Australia caring for adults with cardiac, neurological conditions, COVID-19, and transplant patients as well as other specialties. She is excited to work at Diablo Valley Primary Care and focus on providing high quality compassionate care to those seeking preventative care and screenings as well as those needing care for illnesses, injuries, and chronic conditions. In her free time she enjoys being with friends and family, hiking, running, and cooking healthy meals.

I’m a Board Certified Physician Assistant here at Diablo Valley Primary Care. I’m originally from Las Vegas, Nevada, where I earned my bachelor’s degree in Biology from the University of Nevada, Las Vegas, and my Master of Physician Assistant Studies from Touro University Nevada.

My journey into medicine began in a unique and personal way—I started my career as a professional dancer in a Michael Jackson show. That experience sparked a deep curiosity about the human body and its capabilities, ultimately leading me to pursue a career in healthcare. During my clinical year, I was honored to be a recipient of the HRSA scholarship, an award granted to students who demonstrate a strong commitment to serving their communities and those most in need. It is that mission that continues to guide my work today.

I’m passionate about internal medicine and committed to delivering compassionate, patient-centered care. My goal is always to create a welcoming, respectful space where patients feel heard, supported, and empowered. In my leisure time, I enjoy pilates, snowboarding, going out to eat, and spending time with my family. I’m truly excited to be part of the Diablo Valley Primary Care team!

Please join us in welcoming our new Physician Assistant Jasbeen Rakkar. Jasbeen, a native of the Bay Area, graduated with a Bachelor’s of Science degree in Biology (Physiology) from San Francisco State University. Her journey in the medical field commenced as a clinic manager/medical assistant at an interventional pain management and multi-specialty surgery center. Alongside her professional commitments, she dedicated her time to volunteering at Stanford Health and SF General. Jasbeen’s pursuit of medical education led her to the Physician Assistant program at Dominican University of California, where she held the position of President of the Physician Assistant Student Society. For her, the most fulfilling aspect of her role as a Physician Assistant is providing compassionate care while establishing genuine connections and fostering trust with her patients.

During her time off, Jasbeen finds joy in spending quality time with her family, friends, and beloved dog. Her interests extend to exploring new places, capturing moments through photography, and trying new foods.
