Summary:
Diabetes Educator/Dietitian - Part Time
Concord, NH
The Diabetes Registered Dietitian evaluates, assesses and plans clinical nutrition care specific for persons with diabetes of all ages and populations in accordance with licensure and scope of practice. Educator works in conjunction with the patient's primary care provider to optimize blood glucose control through lifestyle, medication and new updated technology recommendations. Additionally, provides diabetes information to staff, physicians and students as indicated. Reports to the Manager of Diabetes and Nutrition.
Education
Four year degree in Nutrition Science or equivalent and Dietetic Internship.
Certification, Registration & Licensure
Certification required: Diabetes Educator (CDE)
Registration required: Registered Dietitian - R.D.
Licensure required: Licensed Dietitian - L.D.
Experience
Experience working in a clinical setting treating patients with diabetes (can include internship). Two years experience in diabetes education. Ability to work with a multidisciplinary team. Possesses knowledge of growth and development across a diverse age and multicultural population.
Responsibilities
The diabetes educator conducts an assessment of the person with, or at risk, for diabetes using the
assessment process which includes ongoing collection and interpretation of relevant data and the 7
elements of assessment: healthy eating, being active, monitoring, taking medications, problem solving,
reducing risks and healthy coping.
Assumes responsibility for his or her own professional development and pursues continuing education to develop and maintain Diabetes Self Management Support (DSME) knowledge and skills.
The diabetes educator works with the person with or at risk for diabetes to identify mutually acceptable goals.
Maintains a level of productivity that supports patient access and office needs.
Managing workload and processes to meet patient care and diabetes office needs.
Participates in committees, patient discussion and meetings.
The diabetes educator establishes a record of client’s Diabetes Self Management Support (DSMS) experience and follow up DSMS. Group classes are encouraged as appropriate.
Utilizes knowledge and understanding of the profound and prolonged impact of diabetes upon an
individuals’ physical, emotional, social, economic and vocational status.
The diabetes educator develops and carries out a plan that incorporates technology and advanced diabetes education.
Manages insurance and charge requirements.
