Our Capital Area Memory Café had a heartwarming October visit with therapeutic dogs and donkeys — such a relaxing and joyful time! 🐶🫏

Each month, we come together to socialize, enjoy fun and meaningful activities, and learn about valuable community resources. 💬🎨📚

Join us at the Granite Ledges of Concord every third Wednesday from 2–4 PM — it’s a welcoming space for connection and support. 💜

The Season of Remembrance is upon us once again.

We invite you to submit the names of loved ones who have passed to be placed on our online honor roll, so we can all take time to reflect on and remember those we have lost.

Those who choose to donate $250 or more to our Hospice Program between now and Dec. 31 will receive a special hand-blown glass keepsake ornament from our very own Jeff Lamy, juried member of the League of NH Craftsmen.

We will continue to hold our beloved community members in our hearts and memories throughout the season. You can submit loves ones' names here: https://www.granitevna.org/ways-to-give/season/

Dedicated to making a difference through education and leading older adults toward healthier, more independent lives.

For our community health educator Kelli, her role combines everything she's passionate about - public health, social connection and helping people age well in their own homes. She finds that this role perfectly blends her background in both social work and the medical field.

What she loves most about her work is seeing the real impact it has on the community. Hearing directly from participants about how the programs have improved their lives is both rewarding and motivating.

When Kelli's not out in the community, she’s spending time with her son, making memories and taking advantage of everything NH has to offer each season.

We’re so grateful for Kelli's energy and dedication to helping others live and age well.

Brooke is on a mission to make credible, relevant health information accessible to those who need it most.

As a community health educator, she loves having the opportunity to connect with older adults and to build trustworthy partnerships that expand the reach of our wellness programs. She’s passionate about empowering underserved individuals and promoting dignity, independence, and connection.

But it's not just about sharing information - it's about listening, learning and engaging with a population that exudes both humor and resilience.

Outside of work, Brooke takes full advantage of NH’s four seasons - walking trails, kayaking, gardening, or ice fishing, to name a few of her pastimes.

Congratulations to our President and CEO, Beth Slepian, on being named to the National Healthcare at Home 2025 Best Practices and Future Insights Study steering committee.

The committee of 22 industry leaders will guide and oversee a comprehensive study on best practices and future insights for the delivery of home health and hospice care.
