Community Health Integration Position – Cox Family Medicine, PLLC, Rutherford College, NC
Position: Full-time Office-Based Community Health Worker/Certified Medical Assistant (CMA) – Community Health Integration
Location: Rutherford College, NC
Practice: Independent, physician-owned family medicine practice dedicated to compassionate, patient-centered care.

Primary Responsibility:
- Assist patients in addressing social determinants of health (SDOH) issues, such as access to food, housing, transportation, and healthcare resources.
Additional Responsibilities:
- Support community health programs and patient outreach initiatives.
- Collaborate with the chronic care management (CCM), behavioral health integration (CoCM), and transitional care management (TCM) teams.
- Conduct health assessments and support the implementation of care plans.
- Participate in quality improvement efforts to enhance patient outcomes.
Qualifications:
-Current, unrestricted NC CMA or CCMA certification.
- Training or experience in community health work and/or certification and SDOH assessments recommended.
- Experience in primary care or family medicine preferred (new CMAs with a strong fit welcome).
- BLS certification recommended.
- Excellent communication and interpersonal skills.
