Hailey graduated from Western Governors University's Leavitt School of Health with a specialty in Family Nurse Practitioner in 2025. She completed clinical residency rotations in family practice and pediatrics and is trained to treat patients across the lifespan.

Hailey grew up in and currently resides in Morganton, NC. She is married with one daughter, and has a new addition on the way in the Fall. She enjoys cooking and baking, spending time with her family and her church family, and living a simple life rooted in Christ. Her desire to provide higher quality patient care with a foundation in Christ and her Christian values is what led her to our practice. She and Dr. Cox work closely together to serve our patients and aim to provide a memorable and valued experience.

Position: Full-time Office-Based Community Health Worker/Certified Medical Assistant (CMA) – Community Health Integration
Location: Rutherford College, NC
Practice: Independent, physician-owned family medicine practice dedicated to compassionate, patient-centered care.

Primary Responsibility:
- Assist patients in addressing social determinants of health (SDOH) issues, such as access to food, housing, transportation, and healthcare resources.

Additional Responsibilities:
- Support community health programs and patient outreach initiatives.
- Collaborate with the chronic care management (CCM), behavioral health integration (CoCM), and transitional care management (TCM) teams.
- Conduct health assessments and support the implementation of care plans.
- Participate in quality improvement efforts to enhance patient outcomes.

Qualifications:
-Current, unrestricted NC CMA or CCMA certification.
- Training or experience in community health work and/or certification and SDOH assessments recommended.
- Experience in primary care or family medicine preferred (new CMAs with a strong fit welcome).
- BLS certification recommended.
- Excellent communication and interpersonal skills.

Position Overview: Seeking a dedicated PA to join our team, collaborating with physicians and care teams to provide acute, chronic, and preventive care, and participate in CCM, CoCM, and TCM programs.

Key Responsibilities:
- Provide outpatient family medicine care
- Perform health assessments and develop treatment plans
- Collaborate with physicians, RNs, CMAs, and staff
- Engage in preventive care, health education, and quality improvement
- Document in EHR and share after-hours call rotation

Who We’re Looking For:
