🚨 FDA Update on GLP-1 Compounding

The FDA has announced a proposal to exclude semaglutide, tirzepatide, and liraglutide from the 503B Bulks List, stating that it did not identify a clinical need for 503B outsourcing facilities to compound these medications from bulk substances.

This is an important regulatory update, but it is also important to understand what the FDA statement is addressing:

✅ This proposal is focused on 503B outsourcing facilities and the 503B Bulks List.
✅ It does not appear to directly change the 503A patient-specific compounding framework for state-licensed pharmacies.
✅ 503A compounding must still follow existing federal and state requirements, including patient-specific prescriptions, appropriate sourcing, and restrictions on compounding products that are essentially copies of commercially available medications.

The FDA is accepting public comments on this proposal through June 29, 2026.

🧠 Could GLP-1 medications have benefits beyond weight loss?

A new review is adding to the growing conversation around GLP-1 medications like semaglutide and their possible effects on brain health. Researchers reviewed preclinical studies and found that GLP-1 receptor agonists may influence Alzheimer’s-related biological pathways, including amyloid-beta, tau, inflammation, and insulin signaling in the brain.

That is exciting science, but it is important to say this clearly: human clinical data is still developing, and semaglutide is not FDA-approved to prevent or treat Alzheimer’s disease. In fact, recent Phase 3 Alzheimer’s trials of oral semaglutide improved some biomarkers but did not show a statistically significant slowing of disease progression.

So what does this mean for compounded sublingual semaglutide in SubMagna® HMW?

It may increase interest in GLP-1 therapy overall, especially among patients and prescribers looking for needle-free, patient-specific options when clinically appropriate. SubMagna® HMW is designed for sublingual delivery and may support medication contact with the oral mucosa, offering another route for appropriate patients who are working with a licensed prescriber.

TOPICAL PAIN MANAGEMENT, MADE SIMPLE

Richie’s Specialty Pharmacy offers a prescriber-directed topical pain management option with Lipoderm Active-Max® designed for customized patient care.

With this approach, a patient may be prescribed Lipoderm Active-Max® along with a separate capsule formulation to be mixed together at the time of administration, then applied directly to the affected area(s).

Compounding Spotlight: Sumatriptan in SubMagna® HMW

Sumatriptan for migraines and headaches is not new. It has been commercially available for years in dosage forms such as injections and nasal sprays.

What’s exciting from a compounding perspective is the ability to compound Sumatriptan with SubMagna® HMW as a sublingual, under-the-tongue dosage form for appropriate patients when prescribed.

At Richie’s Specialty Pharmacy, we currently have a 25 mg/mL Sumatriptan in SubMagna® HMW formula validated, and we are actively working to validate a 100 mg/mL formula.
