Innovative Family Care, PLLC A primary care family practice centered around you and your family. Office Hours are currently 8-5, Monday - Friday.

At Innovative Family Care, we will provide preventive care and treat acute and chronic illnesses. As a member of our practice, you and your family will be cared for by a team of dedicated health professionals including Dr. Chet Gentry, MD, William Wilson, NP, Kathryn Wayne, NP and Sandra Thompson, NP. We function as a Patient Centered Medical Home. This means our team will act as your medical home. Our goal is to help you be as healthy and fit as possible: mentally, physically, and spiritually. We want to teach you to manage your own health as cost effectively as possible. We will help you keep your full medical history and medical care plan here. No matter what your medical conditions, we will help you develop a medical care plan tailored to your needs. We will assist you in making sure that your medical needs are addressed wherever you are whether it be in the home, ER, hospital, home health, nursing home, or hospice. We strive to manage your care no matter where you go in the health system. We also work together with these different types of care to create a cohesive relationship for you and your loved ones. Dr. Gentry is Board Certified in Family Practice as well as Hospice and Palliative Care. He has over 20 years experience as a pioneer and thought leader in providing excellent care at the local level. He practiced in Sparta for 15 years, providing care to all ages and including OB services. He is experienced in pediatrics, geriatrics, preventive medicine, cardiology, emergency care, hospital care, and ICU. He supervises all of our Nurse Practitioners and assists them when it is necessary so that all of your questions and concerns are addressed.

We will help you keep your full medical history and medical care plan here. Your medical care plan will be tailored to meet your needs and we will work with you to meet your goals. We will assist you in making sure that your medical needs are addressed wherever you are: home, ED, hospital, home health, nursing home, or hospice.
