The Primary Care Clinic of Lehigh County was created as a result of an influx of patient complaints regarding previous experiences of being referred out to numerous specialists for routine chronic disease management. "Why have a P*P if all they are going to do is refer me out to someone else without addressing the issue?" Add to that, the ongoing miscommunication healthcare can generate when multiple providers from multiple hospital and EMR systems are involved in a patient's case. Thus, the Primary Care Clinic of Lehigh County was formed to be the type of medical provider system that collaborates with all providers and ensures all providers collaborate with each other, all of which for the benefit of the patient.
