In the 1970s, newborns with unexplained severe liver problems were added to the list of potential Alpha-1 patients. The diagnostic delay between the onset of symptoms of lung and liver disease and the correct test for Alpha-1 being done still averages a seven year diagnostic delay, largely due to missed opportunities to consider a rare diagnosis beyond much more common causes of lung symptoms due to asthma or COPD.

The vast majority of individuals with Alpha-1 and carriers of a single abnormal AAT gene remain undiagnosed throughout the world. In order to be classified as having Alpha-1, an individual must inherit at least one abnormal AAT gene. Individuals who have one abnormal AAT gene and one normal AAT gene are sometimes referred to as heterozygotes or "carriers." This distinction is important and requires that individuals learn their own AAT genetics.

The alpha-1 antitrypsin protein (AAT) is produced by your liver and helps protect your lungs from damage and irritants like dust, fumes, and smoke. Therefore, there are many individuals with abnormal Alpha-1 tests who do not develop disease if they have lived a healthy lifestyle and never smoked.

The opposite is also true. Without enough AAT circulating in the blood, your lungs are at higher risk for disease with as few as 100 ci******es in a lifetime. You may develop COPD, particularly if you have other risk factors. Therefore, family testing is important to protect those who may not know about their risks.
