Finesse rhinoplasty 🔥

Typically implies a precise but less dramatic change. Clearly, the shape of her birth nose is quite good, but she wanted to decrease the size and improve the symmetry.

We performed a slight lift and deprojection of the tip, without sacrificing the definition or making the nose look wide (common pitfalls of deprojection). This helps reduce the elongated nostril look on side view.

You can also see we narrowed her bones, especially was wider on her right side causing more of a bump on 3/4, and made the symmetry of the tip and nostrils much better.

These cases are quite difficult to do well, as the precision needed to make a good nose even better is really high. ❤️

We don’t post too many patient submitted updates because often the differences in lighting, head angle, and camera / lens settings can be misleading. Even filters are sometimes applied to the afters on some surgeons’ pages. The ethics of such posts should be questioned. The problem is amplified when unrealistic expectations are created for others.

For sure, the best way to judge a before and after is standardized photos / videos. We will generally stick to these formats, but it is nice sometimes to see a more candid / selfie style comparison. In this case, my lovely patient did a really nice job preparing similar images 😍❤️

She’s a bit swollen as you can see (more updates coming) but the improvement in tip and nostril shape already is complimenting her natural beauty. IMO 😁

Correction of alar retraction and bulbous tip 🔥

Alar retraction is one of the most difficult problems to correct in rhinoplasty. The clear cause is a lack of cartilage support in the lateral nostril area. However, many of the corrective techniques (which include lateral crustal struts, repositioning, and both articulated and floating rim grafts) can create new problems with widening / bulk in the tip. In addition, the short and tight skin and internal lining (mucosa) can often prevent soft tissue lengthening this area that’s needed to correct the retraction. These problems are also much worse in a revision, because the scar tissue limits the elasticity of the skin even more.

Here we used llc repositioning and small lateral crural struts to try to find the right balance in optimizing her structure. MTF donor rib was used 🙌🏻😁❤️

DIFFICULT CASE 😅

Super thick 9/10 non-adaptive skin in an overprojected nose. This is probably the most challenging primary configuration, since we want to stretch thick skin for more definition, but the skin volume is already large.

Many complex maneuvers are needed and even deprojecting by 1/2mm too much can cause loss of definition from front and all 3/4 angles. Luckily in her we were able to achieve a 4mm deprojection and still end up with a very aesthetic result ❤️

I always say we shouldn’t aim for the smallest nose possible. Just hacking away at the structure under the skin more and more isn’t going to make it look better and better. If fact, you will need a revision to add back in structure and projection if this is done. Instead, we should make the “smallest nose possible that still looks good”, meaning to reduce up until that limit where any more will look like worse not better. This is the art of rhinoplasty 🔥

6 weeks post op 😍➡️

Compare these photos to my last post to see the type of changes that can occur during the first month of healing. Not all noses are exactly the same, but pretty much it’s always going to feel like a swollen potato in the beginning.

Rhinoplasty is a bit of a leap of faith. You shouldn’t do it if you’re too anxious of a person or if you don’t trust your surgeon and their process. The idea is to be satisfied with the end result and patience is the key 🔑

Are you considering how the skin will adapt to the new structure and proportions?

The bottom line is that this is a very COMPLEX process. Even as a surgeon, it might be difficult to answer these questions accurately. In the surgery, we are using all of our senses, vision of course but also especially tactile sensation (touch). Once I put my hands on the nose, I can understand much better the shape and strength of the underlying structure as well as the skin characteristics overlying it. The skin gets in the way of this understanding sometimes, and photos never tell the whole story (but they do give an idea). Managing the skin is the highest level skill in rhinoplasty, especially in thick skin, scarred noses like this one (she was 9.5/10 with lots of hard scar).
