We are looking for 2 skillful Registered Nurses(RN's) and 2 Licensed Practical Nurses(LPN's). The qualified canadiates will need Home Health experience. The areas the RN's and LPN's will be working are Coral Springs, Coconut Creek, Deerfield Beach, Lighthouse Point, Tamarac, and Margate.
For Registered Nurses doing Medicare visits:
- The Initial Evaluation (aka Start of Care, or SoC)
- RE-Evaluations / Re-Certifications / Discharges (OASIS)
- Follow up visits
- Visits verify between 30mins-1 hour and 30mins
Licensed Practical Nurses:
- Follow up visits
- Visits verify between 30-45mins
Licenses & Certifications:
- Unrestricted RN / LPN license valid in the State of Florida.
- Level II Background Screening (Fingerprinting) completed within the past five (5) years.
- CPR certification
- OSHA certification
- HIV/AIDS certification
- Alzheimer’s certification
- Domestic Violence Prevention Certification
- Medical Error Prevention Certification
- OASIS-E Certification
Insurance & Employment Verification:
- Individual Professional Liability Insurance
- Car Insurance & Vehicle Registration
- I-9 form, with copies of eligibility verification documents (e.g., US Passport or Birth Certificate, or Permanent Resident Card, and Social Security Card).
Health Compliance:
- Proof of COVID-19 Vaccination or exemption form
- Physical Exam from the past six (6) months (can be obtained by the nurse’s doctor)
- Proof of Tuberculosis Vaccination or waiver form
