Care management is essential for improving community health, particularly in underserved areas.
I started my Care Management business because I witnessed the challenges faced by my community—the Black community, the immigrant community, and the nursing community. I knew I had to do something to help. By leveraging my nursing expertise, I aimed to address these problems in meaningful ways.
By focusing on individualized care management and considering social determinants of health (SDOH), Nurse care managers can address the root causes of health disparities more effectively.
This proactive approach leads to more impactful interventions and improved health outcomes. Empowering communities through targeted care management benefits individuals and enhances overall public health.
As nurses, we have a unique understanding of our patients' needs and challenges. In value-based care models, our role is crucial in improving patient outcomes and reducing costs. We have the power to drive significant changes in our communities, creating more health equity and reducing healthcare disparities.
