CareConnect Family Practice, Cordele Providing Primary Healthcare to the whole Family. OPEN Monday-Thursday, 8:00 AM until 5:00 PM, Friday 8:00 AM until 1:00 PM

We provide primary care for children and adults of all ages who have medical conditions or symptoms and complaints. Offering for all ages: annual exams, well-child visits, diagnostics, screenings and referrals to advanced care that meets your medical and personal needs. We offer programs and services focusing on wellness, family, diet, activity and exercise to promote healthy lifestyles.

Our office will be closed Monday, January 19th & we will reopen for normal business hours on Tuesday, January 20th.
