Four Corners Youth Clinics We are a school based health center with clinic sites on the Southwest Open School Campus and the Do The clinic itself is owned by Southwest Open School.

Conveniently located at the junction of 145 and 160 in Cortez, CO on the campus of Southwest Open School (SWOS), the Southwest Open School Based Health Clinic has been providing health services for youth in our community since 1999. The clinic provides year-round integrated health care for ALL youth in Montezuma County and the surrounding area, aged 2-21. Services include screenings, physicals, sick care, reproductive health care and behavioral health counseling as well as providing referrals when appropriate. The clinic employs two nurse practitioners, a licensed professional counselor, a registered nurse, a licensed practical nurse who is also the clinic coordinator and an administrative assistant, all of whom share a passion for working with and serving young people. Services are provided in an integrated care setting, enabling all patients to be seen by primary and behavioral health staff. All providers are employees of Rocky Mountain Youth Clinics, our clinic's medical sponsor.

Our Story

Four Corners Youth Clinics has been in operation as a school based health center (SBHC) on the SWOS campus since 1998. It was established as “Southwest Open School School Based Health Center” and assumed the Doing Business as Name of “Four Corners Youth Clinics” in February, 2018, in preparation for the expansion for expansion of services to the Dolores Schools Campus. SWOS remains the clinic’s lead agency and fiscal agent; it also owns the building in which the clinic is housed. The clinic’s mission is to support the mission of SWOS by offering students access to integrated health services with a focus on prevention and education. Currently, the SWOS campus location serves students attending SWOS and also provides services to all youth, ages 0-21, in Montezuma County. In the 2018-2019 school year, Four Corners Youth Clinics opened a clinic on the Dolores Schools Campus which serves all in the Dolores community up to age 21. The SWOS campus clinic provides year-round integrated health care for ALL youth in Montezuma County and the surrounding area, aged 0-21. Services include screenings, physicals, sick care, reproductive health care and behavioral health counseling as well as providing referrals when appropriate. The clinic employs a nurse practitioner, a licensed professional counselor, a registered nurse, a clinic coordinator, and two administrative assistants, all of whom share a passion for working with and serving young people. Services are provided in an integrated care setting, enabling all patients to be seen by primary and behavioral health staff. Every Child Pediatrics assumed the role of Medical Sponsor for both clinics in November of 2015. As such, they oversee clinic operations and ownership of the medical record, eliminating issues around Health Insurance Portability and Accountability Act (HIPAA) and Family Educational Rights and Privacy Act (FERPA). Every Child Pediatrics is a non-profit organization that provides accessible, affordable, high-quality health care and support programs to children and youth across Colorado regardless of their insurance status or ability to pay.
