We no longer participate with any major medical health insurance carriers. If you have out of network coverage, you will be provided with a "super bill" with all the information needed for you to file a claim for reimbursement.

We continue to participate in Medicare Part B and will bill and receive direct payment from Medicare. You only have to pay your copay and for non covered services at the time services are rendered.

If you have a Medicare Advantage plan, Part C, you will only be charged the Medicare allowed amount for covered services. However, payment is due at the time services are rendered. If you have out of network coverage, you will be provided with a "super bill" with the information needed to file a claim to be reimbursed.

We continue to treat, bill and receive payment directly from insurance carriers for NY State Workers' Compensation and No-fault Auto cases.

There are rumors our practice will be closing January 1st, 2025.

WE ARE NOT CLOSING OUR PRACTICE!

We will be discontinuing participation with Excellus and the NY State Empire Plan as of January 1st, 2025. We will only bill and accept payments for Medicare Part B, No-Fault Auto and NY State Worker's compensation claims as of 1/1/25.

If you have out of network benefits, you will be provided with a "super bill" with the information to submit for reimbursement from your insurance carrier.
