Dr. Djafari Pediatrics Pediatric Practice Our practice monitors information pertaining to these areas on a regular basis to continuously improve in these areas for our patients.

Our practice is making strides to improve in clinical areas such as quality performance, resource use and care coordination, and patient/family experience. Our current measurements for Clinical Quality Performance are:
In 2016 our MMR Immunization rate was 82%, our HPV Immunization was 50%, our Lead Screening was 82%, and our Well Child Visits for Kindergarten age was 85%. In 2017, we increased our MMR Immunization rate to 87%, our HPV Immunization to 72%, our Lead Screening to 84%, and our Well Child Visits for Kindergarten age to 92%. Our medication reconciliation completion percentage was 31.2% for January 1, 2017 to June 30, 2017 with an increase to 90.24% for July 1, 2017 to September 27, 2017. We were able to increase out patient satisfaction percentages in the area of "Ease of getting through on the telephone" from 83% during the time period of June 1, 2017 to July 28, 2017 to 99% during the time period of August 1, 2017 to September 22, 2017. Performance measurement and quality improvement are important areas of concern for our practice, and we continuously work to improve any areas we can.

The program helps eligible households address home health and safety hazards such as lead, radon, and mold at no cost.
