The Mental Health Transitions is Orange County’s first independent mental health transitional living facility. MHT is focused on fostering community, autonomy, purpose, wellbeing, and integration. A true home environment, MHT is not stabilization, it is a safe place to call home for up to a year while community members transition into robust, functional lives. MHT seeks to transform the lives of i

ts community members, along with the healthcare landscape, to promote long term, sustainable support and change. Our vision is to provide a stable home for a community recovering from mental illness where they grow into the rest of their lives. Mental Health Transitions is located in Costa Mesa, CA and is a beautifully designed 4-bedroom home, offering spacious and comfortable living conditions. Each client residing at the Mental Health House will be initially assessed by a therapist and provided weekly therapy sessions for ongoing support. In addition, there will be 24-hour staff supervision, medication management, and comprehensive life skill development education. Client’s will have access to transportation and will participate in weekly house group sessions to progress on working towards their personal identified goals.

MEET OUR TEAM // Chelsea Reeves

Chelsea Reeves, Chief Marketing Officer at Mental Health Transitions, grew up in East Texas where her passion for connecting and helping others was first ignited. Chelsea moved to Newport Beach when she was only 19 to begin her own personal journey of recovery. She began her professional experience over a decade ago when she joined a well-respected women’s program and started her work in the mental health field. Chelsea has worked with other adolescent programs and non-profit organizations in numerous leadership roles and volunteered with the Long Beach Rescue Mission, The Lynne House, Facing Addiction, and actively serves young women in the community of Orange County. Chelsea holds an undergraduate degree in Communication Studies and a master's degree in Marriage and Family Therapy.
