Less than a week until the St. Luke's Mobile Mammogram comes to ACHC! If it is time for you to get a mammogram, St. Luke’s mammogram professionals will be at ACHC this Monday, August 25th, from 10 a.m. to 3 p.m. To get scheduled for a mammogram on the St. Luke's Mobile Mammogram during its visit to ACHC, please call 208-706-2055.

We recommend verifying that St. Luke's is in network for your insurance prior to scheduling an appointment. 💙🤍💙🤍

The next time Southwest District Health (SWDH) WIC services will be at ACHC will be October 15th from 10:30 a.m. to 3 p.m. If you are interested in WIC services, please call SWDH in Weiser at 208-549-2370 or SWDH in Payette at 208-642-9321 to schedule your appointment as ACHC cannot help with scheduling or questions. 💙🤍💙🤍

There are a limited number of in-person appointments available, and they will fill up extremely fast. If you are unable to get an in-person appointment, but need WIC services, SWDH WIC professionals also offer WIC appointments over the phone. To schedule, participants can call SWDH in at 208-549-2370, the SWDH in Payette at 208-642-9321 or SWDH in Caldwell at 208-455-5300 to schedule the appointment. Then a WIC staff member will call the participant at the scheduled time of the appointment. This is for all types of WIC appointments.

Have you heard about ACHC's Diabetes Self-Management Education Program (DSMES) yet? It is ran by ACHC's Sue Warner, FNP-C, CDCES, and she is now offering individual diabetes education sessions on Mondays, Tuesdays, and Wednesdays. People wishing to participate in the program must receive a referral from a provider prior to scheduling an appointment.

ACHC’s DSMES Program's purpose is to help patients learn to manage their diabetes through diet, exercise, medication management, and problem solving. People who are referred to ACHC's DSMES, will also have the added benefit of being monitored and educated personally by Sue Warner while they are in the program. 😁

Did you know that we offer FREE Sports Physicals year round at ACHC? Your child doesn’t even have to be a patient of ACHC to take advantage of this awesome opportunity! So, if your child plans to play sports this coming school year, just call 208-253-4242 to schedule an appointment for a FREE sports physical at ACHC.

We recommend checking with your child’s school to see if they require a specific form for the sports physical. If a form is not brought to the appointment, ACHC will provide one from the Idaho High School Activities Association. 💙🤍💙🤍

Just a reminder! Southwest District Health (SWDH) WIC Professionals will be coming to ACHC to do in-person WIC appointments on Wednesday, August 20th, from 10:30 a.m. to 3 p.m. Please call the SWDH in Weiser at 208-549-2370 or the SWDH in Payette at 208-642-9321 to schedule your appointment.

Less than a week until the monthly Caregiver Support Group! If you take care of another person, whether you get paid for it or not, we highly recommend coming to this Free Caregiver Support Group. The amazing Brennan Hyer, TCC at Home Health & Hospice-West, will be holding the monthly Caregiver Support Group in the ACHC conference room on Monday, August 18th, from 2 p.m. to 3 p.m.

This is a great way to get resources and connect with other caregivers in a positive environment. Also, in addition to all the previous resources this group offered in the past, they will now be able to assist you with Power of Attorney paperwork, and help you gain a knowledge of end-of-life planning. Everyone is welcome! We hope to see you there! Don’t forget, your health is important too! 💙🤍💙🤍

Don’t forget! The Diabetes Prevention Program (DPP) informational session is TODAY, August 12th, at 5 p.m. in the ACHC conference room!

The DPP teaches you how to lose weight and make healthy lifestyle changes to prevent getting type 2 diabetes. So, if you would like to know more about ACHC's DPP and to see if you are eligible for ACHC's no-cost DPP course that is scheduled to start in September, we highly recommend you come to this session today. Nurse Susan Goudie-Adams, ACHC’s DPP Lifestyle Coach, is looking forward to meeting with you and answering any questions you may have about ACHC’s DPP course. 💙🤍💙🤍
