Our Description: The Heart Failure Survival Center of America (HFSCA) is an integrated outpatient cardiac care system that is run and operated by highly qualified staff members who are top providers in general cardiac and heart failure care. HFSCA will serve as a hub destination outpatient cardiac center in the greater Appleton area in Wisconsin and ultimately applying the model to a national level. We are leaders in the development of innovative, cost-effective approaches for prevention and management of cardiovascular disease and heart failure. We can deliver this unique specialty care to our patients by partnering with our patients and the local health care providers. Our ultimate goal is to improve the patient's overall quality of life and to achieve a reduction in heart failure hospitalizations. Our passion is to help our patients not only survive, but to also thrive. The specialty services we offer will be provided and overseen by Dr. Yasser Salem, a board certified physician in Internal Medicine, Cardiovascular Medicine, Heart Failure and Cardiac Transplant. In addition, Dr. Salem has board certifications in cardiac imaging to include Echocardiography, Nuclear Cardiology and Coronary Computerized Angiography. Dr. Salem has additional interest and training in Cardiac Electrophysiology. The combination of such unique certifications makes Dr. Yasser Salem a highly qualified expert in the field of Cardiology, specifically in Heart Failure. This allows Dr. Salem and his team to deliver superior care with confidence in order to achieve superior results for his patients.
