Attention former patients of Dr. Stanley Bise seeking Medical Records Per state and federal regulations, medical records for former patients of Dr. Bise will be kept for 10 years from the date of the patient’s last visit. We do not have access to x-ray films, lab and test results, or reports for surgeries which were done out of office. For those records, please contact the facility the procedure was performed in. We also do not have access to other doctor’s records.
Medical record requests must be made in writing. Please include the full name of the patient, including maiden name if applicable, and date of birth, as well as the dates of service between January 2013 and December 2019 you need copies of. Please also include the address the records should be mailed to and a contact phone number in case any problems arise.
The request must be signed by the patient. If you are a legal guardian or Power of Attorney holder, please note that in the request. Medical records cannot be released to those not authorized by the patient in writing.
Please mail your request to Dr. Stanley Bise, 342 Southgate Dr. Crossville TN 38555. Please allow a few weeks to process the request. Sending a self-addressed stamped envelope may help speed up the process.
