Important Notice for Existing Medicaid Patients:

UnitedHealthcare is no longer under contract with the State of Louisiana. As a result, our clinic can no longer accept this plan.
We sincerely apologize for any confusion regarding plan options. After careful consideration, AmeriHealth is the plan we will work with moving forward.

If you would like to continue receiving care with us, please follow these steps:
1) Contact Medicaid to change your insurance plan to AmeriHealth.
2) After your plan has been updated, call AmeriHealth Customer Service to select or update your Primary Care Provider (P*P) to our clinic.
3) If you experience any issues, please ask AmeriHealth Customer Service to contact our office directly for assistance.

Please note, we are not accepting *new* Medicaid patients, but we are continuing to care for our existing Medicaid patients.
